If you prefer to call, please call (866) 562-0727. You will be connected directly with our Care Coordinator. Please provide the following information:

• Your Name
• Referring Physician's Name, Telephone & Fax Number
• Injured worker's name, home & work telephone numbers, social security number and date of birth
• Injured worker's insurance and employer information
• Diagnosis and frequent & duration of therapy requested

Should you reach our voice mail, please feel free to leave a message and our Care Coordinator will contact you as soon as possible. You may also want to fax the referral to expedite care.
